Upload your vaccine card to Med+Proctor

Per UIndy's COVID-19 vaccine policy, individuals required to upload information to Med+Proctor can use the steps listed below:

Create a Med+Proctor account
  1. Navigate to medproctor.com.
  2. Click on register link.

  1. Enter your full UIndy email address.
  2. Click the Continue button.

You may be redirected to UIndy's single-sign on page. If so...

  1. Log in to your UIndy account with your UIndy username and password.

  1. Enter your Personal Contact Information in all of the required fields.
  2. In the blue box at the bottom, you can select None for Third Party Relationship.
  3. Click the Continue button.

  1. For Requirement Group. Select General Population if you are a UIndy student or Faculty/Staff if you are a UIndy employee.
  2. Your Student/Employee Id (A-number) can be found on your UIndy ID card. If you do not have your ID card, enter 1234 (this is a required field, however data in this field is not important to your account).
  3. For Campus Resident please select the appropriate answer.
  4. Click the Continue button.

End User License Agreement / Terms of Use will appear.

  1. Enter your full name.
  2. Click the Agree button.
  1. Click the Continue button.

COVID Questionnaire

  1. Answer each of these questions as they pertain to you.
  2. Click the Continue button.

  1. Click the Submit your health forms for free without the upgrade button.

You do not need to pay the $10 for the upgrade.


  1. If uploading your COVID vaccination card, select COVID Vaccine. If uploading your exemption form, select COVID Documentation: Optional. If uploading your COVID test results select Supporting Documentation.
  2. Select a file from your preferred location. This will be a picture or scanned copy of your vaccine card or approved exemption form (once they have been approved and sent back to you by Kory Vitangeli).  
  3. Click the Continue button.

Your record is now being reviewed. Do not expedite your review.

  1. Click the Sign Out link.

Once Med+Proctor has reviewed your submitted forms you will receive an email confirmation (from admin@reminders.medproctor.com). This may take a few days.

Med+Proctor support

Med+Proctor is a third-party vendor and not a UIndy system. If you need direct support, you can select their Customer Support on Med+Proctor's homepage.
